DOG DAYCAMP TERMS AND CONDITIONS.
To ensure the health and safety of your dog and all other guests, we require that all clients agree and comply with the following terms and conditions.

I represent that I am the legal owner of the undersigned dog(s). In addition, I certify that my dog meets JFP Doggy Day Camp vaccination standards, is spayed/neutered (if older than 9 months) and has not been exposed to any known communicable disease within 30 days of admission to daycare. I further represent that each time my dog is brought to JFP Doggy Day Camp, I will be recertifying that all the aforementioned is still true.

I certify that all behavioral history and information provided is true and to the best of my knowledge.

I understand that I am not permitted to bring personal items to daycare.

I agree to provide annual updates of vaccinations from my veterinarian. I further agree to notify JFP Doggy Day Camp Staff of any and all changes to health, condition or behavior.

I understand that daycare is offered between 7AM - 5:45PM Monday - Friday. I understand that if my dog is not picked up by 5:45PM, I will be subject to a $10.00 late fee. If you pick up after 6:00, an additional $40.00 will be added.

I understand that the leash-free environment at JFP Doggy DayCamp provides dogs the opportunity to play in close physical contact, including with their teeth and paws. We suggest all dogs have their nails dremeled short, we can provide this service for an additional fee. I acknowledge that no amount of supervision or personalized care by JFP Doggy DayCamp, its agents and employees, can 100% prevent the possibility of injury or illness to my dog.

I understand that JFP Doggy DayCamp reserves the right to refuse admittance to any dog that displays signs of untreated or potentially contagious conditions, demonstrates aggressive behavior, rude repetitive behavior, or fails the standard health and temperament policies as imposed by JFP Doggy DayCamp.

In the event that my pet requires emergency medical care, I agree that JFP Doggy DayCamp employees/agents have the authority to act in my dog's best interest, even if that means seeking emergency medical care prior to contacting me. I further agree to reimburse JFP Doggy DayCamp for all expenses incurred in connection with emergency veterinary care.

Hold Harmless Agreement: By signing this contract, I acknowledge that daycamp at JFP Doggy DayCamp is a cage-free service. I accept the risks involved and agree that I solely am responsible for any damages that result from injuries caused by my dog while at JFP Doggy DayCamp. I agree to indemnify and hold harmless JFP Doggy DayCamp from any and all claims, liabilities, costs and expenses arising out of any harm or injury caused by my dog to other dogs or persons. I (and my agents) expressly waive and relinquish any and all claims against JFP Doggy DayCamp, its owner, employees and agents for any injury, illness, or harm to my dog.
